Case management

Each Veteran we serve will have a case manager who helps identify barriers to housing and builds a personal housing stability plan, then adjusts it as circumstances change.

How intake will work

  1. First contact. Through our outreach, a partner referral, or a Veteran reaching out directly.
  2. Eligibility screening. Confirming Veteran status and whether the Veteran is homeless or at imminent risk.
  3. A case manager. Each eligible Veteran or family is assigned a case manager.
  4. A housing plan. Together, we build a personal housing stability plan.
  5. Connection to services. Rapid rehousing or prevention, plus the supportive services the plan calls for.
  6. Ongoing support. Follow-up and plan adjustments, with the goal of lasting stability.
